Liability Waiver

By registering and signing this form, I hereby give my approval for my child(ren)’s participation in any and all activities prepared by Ms. FangFang Li and Ms. Li Violin Studio. I assume all risks and hazards incidental to the conduct of the activities and release, absolve, and hold harmless Ms. FangFang Li, Ms. Li Violin Studio, and all their respective officers, agents, and representatives from any and all liability for injuries to said child(ren) arising out of traveling to, participating in, or returning from selected activities. I am solely responsible for the safety of my child(ren) before, during, and after lessons, and release Ms. FangFang Li, her family, Ms. Li Violin Studio, and all their respective officers, agents, and representatives from all responsibility for the safety of my child(ren).
